Do these come with the base mount or does that need to be purchased separately? If they need to be purchased separately then what are the part numbers for those bases?

This does not come with the mounting plate or the base. We have a variety of bases and mounting plates on our website. A0-RADI-150, A0-0005-MIN are are few part numbers for the bases. 17-1526-23 and 17-1502-23 are a few part numbers for the mounting plates.
